Living donor liver transplantation with dual grafts -- a case report.

چکیده
BACKGROUND Living donor liver transplantation (LDLT) exposes to risks both the donor, due to a potential small residual liver volume, and the recipient, who faces the risk of small-for-size graft syndrome. In order to overcome these drawbacks, liver grafts from two different donors can be used. This paper presents a case of dual graft LDLT using a right hemiliver and a left lateral section from related donors. CASE PRESENTATION A 14-year old female diagnosed with chronic hepatic failure due to Wilson's disease with Model-for-End- Stage-Liver-Disease score of 25, underwent a dual graft LDLT, receiving a right hemiliver with a reconstructed middle hepatic vein from her sister, and a left lateral section from her mother. None of the grafts complied with a satisfactory graft-to-recipient weight ratio (GRWR), if they would have been independently transplanted. The combined GRWR was 1.10. The donors and the recipient have been followed-up for over 1 year. RESULTS The donors had no postoperative complications. The donors and the recipient were discharged 8 and 19 days after surgery, respectively. After 12-month follow-up, both donors and the recipient were alive, with normal graft function. CONCLUSION Dual graft LDLT can be a feasible solution to overcome the risk of small-for-size graft syndrome. Characteristics of liver grafts in living-donor adult liver transplantation: comparison between right- and left-lobe grafts.
HYPOTHESIS Few studies have investigated the results of research focused on living-donor adult liver transplantation. Different characteristics between right- and left-lobe grafts have not yet been clarified in living-donor adult liver transplantation. Left-lobe graft remains an important option, even in adult recipients. SETTING A single liver transplantation center with a long history of he...
متن کاملCystic Liver Infection after Living Donor Liver Transplantation: A Case Report
There are no reports of cystic liver infection after liver transplantation. Herein, we report a rare case of cystic liver graft infection after living donor liver transplantation (LDLT). The patient was a 24-year-old man with primary sclerosing cholangitis who underwent right lobe graft LDLT. Preoperative abdominal computed tomography (CT) revealed a liver cyst at segment 8 of the donor liver. ...
متن کاملInvasion of Artificial Vascular Graft into Duodenal Bulb after Living Donor Liver Transplantation
Artificial vessel grafts are used for outflow reconstruction in living donor liver transplantation (LDLT). We report a very rare case of invasion of an artificial vascular graft into the duodenal bulb after LDLT. A 54-year-old man underwent LDLT in August 2012. A right hemiliver without a middle hepatic vein was used as a graft. The outflow of segment V was created by anatomizing an artificial ...
متن کاملLiving donor liver transplantation without the use of blood products.
We report on two patients who presented with unresectable hepatocellular carcinoma complicating hepatitis B liver cirrhosis. After evaluation, both patients were accepted for liver transplantation. Being aware of the scarce availability of cadaveric liver grafts and the long waiting time, family members volunteered to be donors for the two patients. Living donor liver transplantation using righ...
متن کاملAdult-to-adult living donor liver transplantation using right-lobe grafts: results and lessons learned from a single-center experience.
Living donor liver transplantation (LDLT) for adults is now a practical alternative to cadaveric liver transplantation. Use of right-lobe grafts has become the preferred donor procedure. Because of the complexity of this operation, a learning curve is to be expected. We report the outcome of our first 41 LDLTs at the University of Colorado Health Sciences Center (Denver, CO). We also discuss th...
